There are many different ebook formats out there. Some of them are supported by specific devices only, others can be read on almost any reader or with a free app.
Word files, PDFs, ODT, RTF, TXT and HTML all convert here. What comes out is a real ebook: the text reflows to whatever screen it is opened on, so the reader can change the font size without the layout falling apart.
Two things are worth filling in before you start. The title and author fields decide how the book appears in a library. Without them it inherits the filename. And if the source is an older text file, the input encoding setting prevents accented characters turning into nonsense.

EPUB. It is the open standard and it opens on Kobo, Nook, Apple Books, Google Play Books and almost every reading app. Modern Kindles accept it too, sent through Amazon's own delivery service.
Yes. DOC and DOCX both convert, as do ODT, RTF, TXT, HTML and PDF. Headings become chapter markers, so the finished book has a working table of contents.
Because a reader's library sorts by them. A file converted without them shows up under its filename, which is rarely the name of the book.
Headings, paragraphs, bold and italics, lists, links and images all carry over. Fixed page layouts do not, and they should not: an ebook reflows so the text fits whatever screen it lands on.

The source file most likely declares the wrong character set, which older TXT and HTML files often do. Set the input encoding before converting and the characters come through correctly.
